Mentor on Discharge
What is "Mentor on Discharge"?
The MoD (Mentor on Discharge) program is a NAMI ACS registered program developed with a Mental Health Services Innovation Grant in 2010. The program has shown to reduce the re-hospitalization rate of psychiatric patients in an acute care psychiatric hospital by over 70%. The program was suspended in 2019 due to lack of funding by Alameda County Behavioral Services.

For the benefit of patients at a reduced cost to funders, NAMI Alameda County South continues to seek reestablishing this program.
